My htv keeps separating from the adhesive backing. Sometimes it’s when I’m weeding. Sometimes it’s after it’s been pressed or washed. Is it too old to use? Or just poor quality?

Your htv stick together may be because you are using PVC material. PVC HTV are generally thicker and less expensive. Being thicker means they’re less flexible and don’t quite have as soft a hand as PU. The products in our shop are all made of PU.
